What forms do I need to complete?
Three, and all of them can be done online:
- Patient health history. Medical conditions, medications, allergies, pediatrician’s name, and birth history for infants. About six minutes.
- Consent for treatment and dental materials. Includes the fluoride varnish consent, which you may decline without any awkwardness.
- HIPAA acknowledgment. Confirms you received our Notice of Privacy Practices.
If you would rather fill them out on paper, arrive fifteen minutes early and we will hand you a clipboard.
What should I bring to the first visit?
- Your insurance card, or your CareSource or Buckeye card
- A photo ID for the adult accompanying the child
- A list of current medications and doses
- Any recent x-rays or records from a previous dentist
- A comfort object, if your child has one
Who can bring my child?
A parent or legal guardian must consent to treatment. After that, a grandparent, nanny, or other named adult can bring your child to appointments as long as you have listed them on the consent form. You can add someone by phone.
For divorced or separated parents, we will need documentation of who holds medical decision-making authority. We keep both parents’ contact details on file and either may be given clinical information unless a court order says otherwise.
Transferring records from another dentist
Sign the release at your first visit and we will request them. It usually takes about two weeks. Do not delay your appointment waiting for records to arrive; we will examine your child either way and update the chart when they come in.
Do you need anything for a sedation appointment?
Yes, and it is different. You will get a separate sedation consent and written fasting instructions at the visit where sedation is scheduled, and we will call you the day before to confirm you have them and to answer questions.
